Mobilizing for Action Through Planning and Partnerships (MAPP)
MAPP is a process by which the Ocean County Health Department will facilitate the development of a Community Health Improvement Plan. This plan will prioritize health indicators and issues in our community and will provide a roadmap by which future program development will be conducted. Community Health Improvement Plan (CHIP)
“The Community Health Improvement Plan (
CHIP) reflects the top six health priorities within Ocean County. The health priorities were derived from the strategic health planning process known as Mobilizing for Action through Planning and Partnerships (MAPP). Subcommittees have been developed to work on each priority issue and have developed action plans in order to resolve or make a positive impact on these issues. The CHIP is designed to be utilized as a resource document for agencies looking for comprehensive information on the health needs of Ocean County residents.
The Health Department collaborated with numerous stakeholders, community residents, and agencies within the County over a three year span. The cooperation with these community partners made this effort successful.”
