ACCAP

The Ocean County Health Department provides case management for adults and children in the AIDS Community Care Alternative Program (ACCAP).  The purpose of ACCAP is to help financially and medically eligible individuals remain in the community rather than transferring to a nursing facility or hospital setting.  Case management services involves  planning,  coordination and monitoring of various services in order to best serve the patient.  (See Case Management Services)  For more information, please call 732-341-9700, extension 7603.
