MEET OUR HOME HEALTH CARE TEAM

Director Public Health Nursing
NURSES
Our experienced, compassionate home care, registered nurses, are experts who are skilled in caring for patients in the comfort of their own homes in order to promote optimal recovery from episodic illnesses and prevent further decline. Our scope of services includes:
  • Assessment of the physical, functional, psychosocial, spiritual, environmental, and nursing needs of the patient/family
  • Collaboration with the physician in establishing a treatment plan
  • Provision and demonstration of nursing procedures, techniques, and skills
  • Teaching and counseling for health promotion/ illness prevention, illness treatment, and recovery
  • Identifying the need for additional community services and initiating appropriate referrals
  • Promotion of continuity of care through coordination of patient care services
  • Ongoing evaluation of patient/family response to nursing care as well as supervision of home health aides and/or caregivers in the provision of care
Examples of specific types of nursing  includes:
  • Pain management
  • Wound care treatment and consultation services of our certified wound care specialists
  • Medication teaching and management
  • Ostomy care and teaching
  • Rehabilitative and restorative care following surgery
  • Injectable medication administration
  • Disease management and treatment for conditions such as diabetes, heart disease, cancer, pulmonary disease, renal failure, stroke, urological problems, hypertension, and neurological impairments
  • Maternal child care to at risk individuals/families
HOME HEALTH AIDES
Our aides are certified by the NJ Board of Nursing as caregivers dedicated to meeting the personal care needs of our patients under the supervision of the Registered Nurse.  Examples of the support services they provide include:
  • Bathing, oral hygiene, grooming, and dressing
  • Light exercise, transfer assistance, and walking
  • Meal preparation
  • Shopping and errands
  • Laundry and light housekeeping
  • Personalized care, including medication reminders for medication that is self administered
MEDICAL SOCIAL WORKERS
Social workers are available to address the social and emotional needs of patients, families, and significant others as related to the patient’s illness.  Their goal is to work short term with or on behalf of the patient/family to and efficiently remove those barriers which interfere with effective medical treatment or the patient’s rate of recovery.
Their interventions may consist of:
  • Taking action to obtain appropriate community services and resources for resolving a patient’s problem
  • Counseling services that may be required by the patient or patient-family group
  • Assessment of the social, emotional, financial, and support system factors as related to the achievement of patient care goals and outcomes
PHYSICAL THERAPISTS
Working closely with the patient’s physician, registered physical therapists design and provide rehabilitative services according to the patient’s specific clinical needs.  They are experienced in caring for all types of injuries and conditions, including interventions related to joint replacement surgery, cardiac, pulmonary, neurological, orthopedic, and musculoskeletal problems. Skilled physical therapy can include:
  •  Assessment of need for in-home adaptive equipment and devices to enhance safety and independence
  •  Establishment and provision of a theraputic home exercise program
  •  Gait, balance, strength, and transfer training
  •  Evaluation of baseline and rehabilitation potential
  •  Instruction to caregivers on transfers, ambulation, equipment use, and exercises to maximize functional ability
 
OCCUPATIONAL THERAPISTS
Our licensed occupational therapists are available to assist patients in acquiring optimal independence in carrying out their routine activities of daily living, often making use of adaptive interventions to address physical, developmental, cognitive, or emotional limitations.  Skilled occupational therapy may include:
Teaching compensatory techniques to improve the level of independence in activities of  daily living
Selecting and teaching task oriented therapeutic activities designed to restore physical function
Designing, fabricating, and fitting of self-help and adaptive devices
Supervising others, including caregivers, in the execution of therapeutic activity programs
 
 
SPEECH THERAPISTS
Licensed speech-language pathologists work with patients and families to provide individualized  speech therapy services in the home to improve speech, expressive and/or receptive language, and oral-motor function such as swallowing.  They assess for, provide, and teach selected rehabilitative or developmental procedures as ordered by the physician.  Patients who have experienced neurological damage, injury to the head and neck, strokes, developmental delays, or loss of hearing are frequently candidates for speech therapy programs. Examples of skilled speech therapy are:
  • Assessment of a patient’s rehabilitative needs, including the causal factors and the severity of the speech, language, and/or swallowing disorder
  • Services directed toward specific speech/voice production due to deficits caused by illness or injury
  • Training the patient, family, or caregivers to augment the speech-language communication or  treatment program
  • Assisting the patient with aphasia in the rehabilitation of speech and language skills
  • Assisting a patient with a voice production disorder to develop proper control of the respiratory and vocal systems for optimal voice production

 

REGISTERED DIETICIANS
Registered dieticians offer nutritional assessments and counseling to patients who present with particular conditions for which proper nutrition plays a key role. Individualized recommendations and plans are developed and taught that reflect personal preferences and lifestyle.  Examples of the kinds of conditions that our dieticians address are:
  • Diabetes
  • Renal disease
  • Cancer
  • Weight problems
  • Arthritis
  • Cardiac disease
